Our Referral Process

1. A preliminary telephone consultation will take place, to discuss current vacancies and prospective young person’s support needs.
2. Completion of a RockStone internal assessment form, in order to assess risk and support needs and how soon the young person needs to be placed, by the placing authority. The RockStone internal assessment form, is then evaluated internally by Rockstone.
3. The placing authority to send over the following; • A RockStone internal assessment form mandatory • A risk assessment (completed by local authority) mandatory • An essential information handover • Placement Proforma
4. 4. Agreement on funding arrangements for the placement.
5. Rockstone will then invite the young person to attend an initial visit to the placement (accompanied by a professional). The young person will be interviewed so that Rockstone can get an impression of the Young Person. A unit tour will then be carried out so the young person can see their new potential living space.
6. Upon the young person’s arrival at the placement to move in, they will receive an induction which will include; • agreeing to house rule • signing license agreement • completing risk management plan. • Booking first key work session, which should take place within 48 hours of moving into the placement in, order to complete the support plan.
